Medication-related morbidity and mortality are significant health concerns due to unsafe medication use practices, which includes polypharmacy and inappropriate prescribing. Reducing medication errors and improving prescribing practices are national priorities. The Houston Medication Safety Symposium will discuss the latest advances in deprescribing in older patients and prescription drug monitoring programs to improve safe medication use practices.
